Chief Population Health Officer Remote (anywhere in United States)

The Company is in search of a Chief Population Health Officer to provide strategic and clinical leadership in the areas of population health and value-based care. The successful candidate will be responsible for developing and overseeing value-based care strategies, including provider contracts, reimbursement models, and clinical quality programs. This executive will also lead the expansion of clinically integrated networks and partner with health system and provider leadership to design and implement sustainable value-based care programs. A key aspect of the role is to translate clinical, operational, and financial objectives into scalable population health strategies and to establish frameworks that improve quality, outcomes, and the total cost of care. Applicants for the Chief Population Health Officer position at the company should have significant executive-level experience in value-based care and population health, particularly within the provider ecosystem.
