Vice President of Care Transitions & Throughput

The Company is seeking a Vice President of Care Transitions and Throughput to improve care coordination, discharge planning, and patient flow across acute and post-acute settings. The executive will standardize throughput workflows, lead interdisciplinary teams and cross-functional initiatives, and develop partnerships with post-acute providers. Responsibilities include overseeing transfer coordination, using operational data and technology to guide capacity planning, and improving measures such as length of stay, readmissions, discharge efficiency, and patient experience. The role also includes managing care progression costs, authorization and denial processes, and building a collaborative, accountable, patient-centered culture. Candidates must hold a bachelor’s and master’s degree; a master’s in business, health administration, or nursing is preferred. At least 10 years of progressive leadership experience in care management is required, along with demonstrated success improving care transitions, discharge planning, length of stay, and utilization across a complex, multi-site health system. Applicants should understand payer partnerships and be skilled in communication, project management, strategic planning, and analysis of clinical, operational, and financial data. The role also requires the ability to lead independently and foster collaboration across multidisciplinary teams.
